Student Forms

The Student Forms feature lets you complete and submit various forms. Examples might include:

  • Leave of Absence

  • Change Major

  • Application for Graduation

  1. From the Student Forms feature, click on the appropriate form link.

  2. Complete the form. You can navigate through the form by clicking the Next Page--> and/or <--Previous Page links. The confirmation screen notifies you of any errors in the questions for each section. After you correct the errors in each section, you can submit the form.

    Tip

    If the form is configured to allow you to finish filling in the form at a later time, you can click the Save and Finish Later button. The next time you enter the Student Requests feature and click on the form link, the form will open—pre-populated with the data values saved.

  3. When you are finished, click Submit Form.

  1. From the Student Forms feature, click on the form link.

  2. Complete the form. You can navigate through the form by clicking the Next Page--> and/or <--Previous Page links. The confirmation screen notifies you of any errors in the questions for each section. After you correct the errors in each section, you can submit the form.

  3. When you are finished, click Submit Form.

Note

This option may not be available for all forms.

  1. From the Student Forms feature, click on the form link called My <form name> submissions.Your submissions are displayed.

  2. Do one of the following:

    1. Click Continue this saved form or the Continue your saved <form name> link if you have an incomplete form that you wish to finish.

    2. Click Fill out a new <form name> link if you wish to start a new form.

    3. Click the View Submission link to view a previously completed submission.

  3. Click Back to Main Screen if you wish to return to the main screen.
